Rinse the rice under cold water until the water runs mostly clear, then drain well.
In a small saucepan, combine the rinsed rice, water, and a pinch of salt, then bring to a gentle boil over medium heat.
Reduce heat to low, cover, and simmer until most of the water is absorbed and the rice begins to soften, about 8–10 minutes.
Stir in the milk and lemon zest, return to a gentle simmer, and cook uncovered over low heat, stirring often so it does not stick.
When the rice is tender and the mixture is thick and creamy, stir in the honey, sugar if using, vanilla, and butter until fully dissolved and smooth.
Taste and add a small squeeze of lemon juice if you want more brightness, then remove from heat and let stand 5 minutes to thicken slightly.
Serve warm or chilled in a small bowl for one, optionally drizzling a little extra honey on top before serving.
